Welcome to CheckApp, the modern platform connecting patients, medical professionals, and facilities in one unified experience.
This guide walks you through using all features to make the most of your day. Always keep CheckApp on top of your mind, the seamless way to manage your health.
- Patient Account Registration
- Go to https://www.checkapp.now using your preferred web browser.
- On the homepage, click the Create Account button to begin the account setup process.
- Choose Personal Account.
- Enter your First Name and Last Name as it should appear in the system.
- Provide a valid and active Email Address and Phone Number that you can access for verification.
- Enter your Birth Day and Gender to complete your registration information. Click Next.
- Enter your preferred home Address as it should appear in the system. Click Next.
- Enter your preferred Username following the rules indicated.
- Create a secure Password also following the rules indicated, then re-enter it to confirm accuracy.
- Remember these security credentials as they are the details to be used for you to login. Click Next.
- Read the End User License Agreement (EULA) carefully and tick the checkbox to confirm. Click Next.
- Read the Data Privacy Policy carefully and tick the checkbox to confirm. Click Next.
- Wait for the system to verify your registration. Once verified, the app will redirect you to the page where you are
requested to log in using your registered credentials. Congratulations and welcome to the CheckApp community.
- Logging In
- Go to https://www.checkapp.now using your preferred web browser.
- Locate the Sign In section on the homepage.
- Enter your registered Username in the designated field.
- Enter your secure Password carefully to avoid login errors.
- Double-check that your credentials are correct.
- Click the Sign In button to access your account.
- If the details are correct, you will be redirected to your personal CheckApp Dashboard.
- If you forget your passkey, click Forgot Password to reset it via email.
- Changing Your Avatar
- Go to https://www.checkapp.now using your preferred web browser.
- Log in to your CheckApp account.
- Click on the green circle image or initials on the upper right corner of your dashboard.
- Click Upload Avatar.
- Click on the plus button to upload new photo or image.
- Select the image photo you wanted to use as avatar.
- Click Save Changes.
- Confirm system change prompt.
- Click X button to close.
- Click < Edit Profile button to return to your dashboard.
- Updating Your Personal Information
- Go to https://www.checkapp.now using your preferred web browser.
- Log in to your CheckApp account.
- Click on the green circle image or initials on the upper right corner of your dashboard.
- Click Edit Info.
- Change personal information details as accurate as possible. Some of these information are to be shared to
your attending medical professional on your medical consultations.
- Click Save Changes.
- Confirm system change prompt.
- Click < Edit Profile button to return to your dashboard.
- Changing Your Password
- Go to https://www.checkapp.now using your preferred web browser.
- Log in to your CheckApp account.
- Click on the green circle image or initials on the upper right corner of your dashboard.
- Click Change Password.
- Enter Current Password. This is required to ensure security.
- Create a secure Password also following the rules indicated, then re-enter it to confirm accuracy.
- Click Update password.
- Confirm system change prompt.
- Click < Change Password button to return to your dashboard.
- Hiding and Unhiding Weather Indicator
- Go to https://www.checkapp.now using your preferred web browser.
- Log in to your CheckApp account.
- Click on the green circle image or initials on the upper right corner of your dashboard.
- Toggle Show Weather to turn it on or off.
- Personal Medical History
- Go to https://www.checkapp.now using your preferred web browser.
- Log in to your CheckApp account.
- Click on the green circle image or initials on the upper right corner of your dashboard.
- Click Medical History.
- Update your personal medical history by adding items: Medical History, Allergies, Medications,
Vitals, Vaccinations.
- Update your medical details as accurate as possible. Some of these information are to be shared to
your attending medical professional on your medical consultations.
- Click < Medical Records button to return to your dashboard.
- Family Medical History
- Go to https://www.checkapp.now using your preferred web browser.
- Log in to your CheckApp account.
- Click on the green circle image or initials on the upper right corner of your dashboard.
- Click Family Members.
- Click Add Family Member to include and add all pertinent personal information.
- Click Save Changes.
- Confirm system change prompt.
- You may also customize their avatars for easy reference.
- You may also edit and deactivate family members if opted.
- Click Records on family member list to update family medical information.
- Update your family member medical history by adding items: Medical History, Allergies, Medications,
Vitals, Vaccinations.
- Update your family member details as accurate as possible. Some of these information are to be shared to
their attending medical professional on their medical consultations.
- Click < [Family Member's name] Records button to return to your dashboard.
- Creating an Appointment to Your Medical Professional
- Go to https://www.checkapp.now using your preferred web browser.
- Log in to your CheckApp account.
- Click on the blue circle with plus symbol on the lower right corner of your dashboard.
- Select New Appointment.
- Select Medical Facility, Specialization and Medical Professional of choice.
- Information of the facility and the attending medical professional are listed to guide you on your appointment schedule.
- Choose patient. If you have added family members as well into your account, their names will also show in the list.
You may be able to select them if they are the patients who wish for the appointment.
- Remember the one patient, one schedule rule. A separate appointment request should be made for a family member if
they will also be setting an appointment with the medical professional.
- Select the date of appointment and choose a consultation mode.
- You may also indicate the reason for your visit if opted.
- Click Book Appointment once done.
- Confirm system change prompt.
- Congratulations. You may now see your appointment request on your dashboard.
- Checking the Status of Your Appointments
- Go to https://www.checkapp.now using your preferred web browser.
- Log in to your CheckApp account.
- On your dashboard is a list of appointments you booked from the start of your registration in CheckApp
arranged from the most recent to the least.
- You may also filter your appointments by clicking on the desired statuses on the top of the list.
- Always make a habit to check the status of your appointments and their details.
- Details of your appointment includes:
- medical professional
- medical facility
- date of appointment
- patient name for appointment
- status
- Different statuses are shown in the list for your appointment guidance and are as follows:
- PENDING - appointment is still to be confirmed by medical professional or secretary
- CANCELLED - appointment is cancelled due to reasons indicated
- CONFIRMED - appointment is confirmed by medical professional or secretary
- ARRIVE - medical professional, secretary or patient arrived at clinic
- IN-PROGRESS - patient is now with the medical professional at clinic
- COMPLETED - appointment marked as completed or finished by medical professional or secretary
- Patient may also view the complete details of their appointment by clicking the View button.
- Cancelling A Scheduled Appointment
- Go to https://www.checkapp.now using your preferred web browser.
- Log in to your CheckApp account.
- On your dashboard is a list of appointments you booked from the start of your registration in CheckApp
arranged from the most recent to the least.
- You may also filter your appointments by clicking on the desired statuses on the top of the list.
- Though appointment cancellations are inevitable, kindly be responsible and cautious of the reasons
why scheduled appointments are to be cancelled by a patient.
- Choose which scheduled appointment is to be cancelled.
- Click Cancel button to cancel appointment and possibly indicate the reason for canceling.
- Click Submit button to proceed.
- Confirm system change prompt.
- Re-booking A Scheduled Appointment
- Go to https://www.checkapp.now using your preferred web browser.
- Log in to your CheckApp account.
- On your dashboard is a list of appointments you booked from the start of your registration in CheckApp
arranged from the most recent to the least.
- You may also filter your appointments by clicking on the desired statuses on the top of the list.
- Choose which scheduled appointment is to be re-booked.
- Click Rebook button to rebook a previously scheduled appointment.
- Select the date of appointment and choose a consultation mode.
- You may also indicate the reason for your visit if opted.
- Click Book Appointment once done.
- Confirm system change prompt.
- Congratulations. You may now see your re-booked appointment request on your dashboard.
- Editing A Scheduled Appointment
- Go to https://www.checkapp.now using your preferred web browser.
- Log in to your CheckApp account.
- On your dashboard is a list of appointments you booked from the start of your registration in CheckApp
arranged from the most recent to the least.
- You may also filter your appointments by clicking on the desired statuses on the top of the list.
- Please be reminded that confirmed appointments can no longer be edited to avoid confusion
on the side of the medical professional or the secretary.
- Choose which scheduled appointment is to be edited.
- Click Edit button to edit a pending scheduled appointment.
- Edit the appointment details as opted.
- Click Save Changes once done.
- Confirm system change prompt.
- Congratulations. You may now see your edited appointment request on your dashboard.
